Palliative care is a resource for anyone living with a serious illness. Our goal is to enhance your quality of life by managing your symptoms without the disruption of office visits or hospital stays.

Together, we’ll create a care plan that matches your individual goals, values, and beliefs. We focus on symptom management, advance care planning, and coordination of care with your primary care physician.

Palliative care is an extra layer of support that works side by side with your current health care team so you can focus on what is most important to you.

Palliative visits are typically scheduled around 2 weeks to 8 weeks apart. The palliative nurse practitioner will visit you at home to complete an assessment and make recommendations.

While the nurse practitioner does not write prescriptions, by request, she will send the visit notes to your primary care physician to share observations, recommendations, and any discussions about goals of care. The palliative team works with a social worker and can request visits and resources based on needs.

Palliative care is an extra layer of support that works side by side with your current health care team.

Once a visit is scheduled…

If you are scheduled for a visit, please have your current medical diagnoses and current medication list with you. You may have family members with you at the appointment if you wish. A visit will typically last for about an hour.

Our Team

Dr. James Boal

CHIEF MEDICAL DIRECTOR

Dr. James Boal has been a part of the Angela Hospice team for over 20 years, starting as Medical Director in 2000, then becoming Chief Medical Officer in 2017.

A graduate of Wayne State University’s College of Medicine – where he is now a Clinical Associate Professor – Dr. Boal is a member of the Michigan HomeCare and Hospice Association, National Hospice and Palliative Care Organization, and the American Academy of Hospice and Palliative Care.

Double-board certified with the Hospice Medical Director Certification Board, and American Board of Family Medicine, Dr. Boal’s work outside of Angela Hospice includes teaching engagements across the state, educating the next generation of hospice doctors, and speaking engagements covering all things hospice and palliative care.

Jamie LaLonde

EXECUTIVE DIRECTOR

Prior to becoming Angela Hospice’s Executive Director in 2022, Jamie LaLonde spent the last 15 years working with the geriatric population in residential settings. Holding various roles, her last six years as an administrator allowed her to use and develop skills to support her love of program development, mentoring teams and supporting older adults. LaLonde is a licensed Master of Social Work and earned a Graduate Certificate in gerontology through Eastern Michigan University.

She completed the Nursing Home Administrator Program from Ferris State University in August 2020 and also holds instructor status for several different modalities of dementia care education and programming. LaLonde most recently became an adjunct lecturer at the University of Michigan’s School of Social Work. She has also volunteered with the Alzheimer’s Association, Arbor Hospice, and the University of Michigan Geriatrics Center.

Lisa Little

PALLIATIVE CARE PROGRAM COORDINATOR

Lisa Little started as an intern at Angela Hospice before transitioning into her role as Palliative Care Program Coordinator in November 2023. Prior to Angela Hospice, Lisa worked as an occupational therapy assistant at St. John Macomb Hospital and Novi Lakes Health Campus/Synchrony Rehabilitation. Her work in occupational therapy is where she discovered her love and passion for working with the older adult population, and lead to her dedicating her career to that field.

Lisa is near the completion of her Master of Public Health at Purdue Global University, where she received her Bachelor of Health Sciences, Healthcare Administration in 2021. Prior to that, Lisa went to Macomb Community College and earned an Associate of Applied Science, Occupational Therapy Assistant. She is currently certificated with the American Red Cross: CPR and First Aid Provider, an NBCOT Certified Occupational Therapy Assistant, and has her Michigan Occupational Therapy Assistant License. Lisa also runs Aging with Grace Outreach, a ministry that works to provide relevant research for older adult advocacy and health policy efforts, and to reach the older adult population in need within nursing facilities.

Jordan Richter

NURSE PRACTITIONER

Jordan Richter joined the Angela Hospice team in August 2021 as a Nurse Practitioner, and in 2024 became their Director of Angela Hospice at Lourdes. She received her Master of Adult-Gerontology Primary Care Nurse Practitioner with a Hospice and Palliative Specialty from Madonna University in August 2021. She is board certified in hospice and palliative care.

From 2014 to 2021 Jordan worked at St. Mary Mercy Hospital and Ascension Providence Hospital, where she was an assistant clinical leader and charge nurse in the Intensive Care Unit. The University of Detroit Mercy alumna – where she earned her Bachelor of Science in Nursing with an Addiction Studies Minor – currently volunteers for the Motor City Mountain Bike Association and the National Mountain Bike Patrol. She’s also a member of American Nurses Association and the American Association of Nurse Practitioners.
